MSYSA Registration Procedures

These procedures provide Olney NCSL and OBSL teams information on registering teams for the 2008/2009 seasonal year which starts September 1 and goes through August 31, 2009 and covers both the fall and spring seasons.


USYSA 2008/2009 Age Groups

  • U19 August 1, 1989 to July 31, 1990
  • U18 August 1, 1990 to July 31, 1991
  • U17 August 1, 1991 to July 31, 1992
  • U16 August 1, 1992 to July 31, 1993
  • U15 August 1, 1993 to July 31, 1994
  • U14 August 1, 1994 to July 31, 1995
  • U13 August 1, 1995 to July 31, 1996
  • U12 August 1, 1996 to July 31, 1997
  • U11 August 1, 1997 to July 31, 1998
  • U10 August 1, 1998 to July 31, 1999
  • U09 August 1, 1999 to July 31, 2000
  • U08 August 1, 2000 to July 31, 2001

Registration forms, the current database program and corresponding instructions are available on the MSYSA website. Click the "Player Registration" tab. Then click the link on the word "download" in the paragraph that starts with "Version 2 of the Registration Database Program". ALL Olney NCSL teams and U09 and U10 OBSL teams must use the database program in preparing their initial roster and player passes.

Some players on teams may be considering playing on multiple teams this season. The players and their parents should be aware that some clubs and leagues have rules prohibiting travel players from participating in recreational or house programs.

Registration Procedure

Go to the MSYSA website, click on the "Player Registration" tab and print sufficient copies of the current version of the following forms for each player and adult official:

  • "Membership and Player Registration Form" for each player
  • "Player Rights & Obligations" for each player
  • "Adult Official Registration Form" for each team official

Download the database program and program instructions. If you have problems, follow the instructions on the website for help.

Have the parents complete the Membership and Player Registration Form and retain the Player Rights and Obligations form for their information. If the player does not have a social security number, contact your registrar and the registrar will provide you with an ID number. If a player is also playing on a house or recreational team, he/she is NOT rostered to another team. A player is only rostered to a travel team.

If a player is primary rostered to another travel team and is multiple rostered on your team, contact your registrar and the registrar will provide you with his/her multiple roster ID number. The ID number is used in lieu of the social security number and a gold Multiple Roster player pass must be used.

If the player resides outside of Maryland (usually the District of Columbia or Virginia), you must download the MSYSA/VYSA Player Registration and Permission to Play Across State Line form from the MSYSA website AND have the form and player passes validated by a VYSA registrar prior to submitting your team package. Call your registrar for details.

The E-Mail address is optional. All other information must be completed. If the family does not have medical insurance, they must check the no insurance box. Both the player and parent/guardian must sign the form.

OBSL requires two adult officials be listed for each of our teams. Coach and manager are mandatory. If the coach has another team, an assistant coach must also be listed.

Have each adult official complete and sign the Adult Official Registration Form.

For adults, the name is not an issue. James Madison may be listed as Jim Madison. For players, the name is a significant issue. The name, specifically the first and last name, must be entered into the team database exactly as it appears on the proof of age document and will then print accordingly on the roster and player passes. The following ORIGINAL DOCUMENTS are accepted as valid proof of age and must be submitted for each player at the time of registration. Notarized copies or uncertified photocopies are not accepted.

The Department of Vital Statistics or similar agency in the state or county of birth to be acceptable must certify copies. Hospital or baptismal records are not acceptable. THERE ARE NO EXCEPTIONS TO THESE RULES. If the family does not have an original birth certificate, with the use of a credit card expedited service is available in obtaining these documents by calling Vitalchek at 1-800-255-2414.

The acceptable documents are:

  • Birth certificate
  • Uniformed Services ID and Privilege Card (DD 1173)
  • Board of Health records
  • Passport
  • Alien registration card issued by the US Government
  • INS certificate attesting to age
  • Certificate of American citizen born abroad issued by appropriate agency
  • USYSA Player Pass from prior seasonal year
  • Valid driver’s license

Registrars may request additional documentation in English to verify foreign language documents. The registrar simply looks at these documents to confirm the player’s name and date of birth and returns them to the team for the manager to return to each family. Registrars do retain player passes from the prior year.

Do not use a player’s middle name or middle initial.

As an example, player’s birth certificate lists his name as Paul Joseph Breitner. He does not use his first name and is called "Joey". He must be entered into the team data base as Paul Breitner and Paul Breitner will be the name that appears on the roster and player passes AND he must sign the back of the two player passes as Paul Breitner. The first and last name on the birth certificate/passport must be the same as the name on the roster, player passes and the player’s signatures on the back of the two passes. THERE ARE NO EXCEPTIONS.

If you used a child’s middle initial last year, do not use the middle initial this year. The child’s player pass from last year will list the name as Sean D. Jones. This year the name/signature will be Sean Jones.

In some rare instances of adoption/remarriage a child’s name is legally changed. If John Paul Jones has had his name legally changed to John Paul Jefferson, the registrar will need both the birth certificate and the court order changing his name and he will be listed as John Jefferson.

The player must sign his/her name. Some U09/U10 players are very young. Neatness is not a criteria. If the player is not writing cursive, have him/her print their name. The idea is that it is the player’s signature, not a parent. They also must sign the Player Registration.

To make it easier for the players to sign their names, you may draw a line on the back of the Membership Card on top of "US" and "FIFA Fair Play" to indicate where the player signs. The player will be signing on top of the printed lettering, which is where they are supposed to sign on the membership card. The line for player signature is imprinted on the back of the player pass. The player has to sign the back of BOTH cards.

You also need two current player photographs (1 inch by 1 inch, front view) which must be glued (taping, stapling, or clipping are not acceptable) to the back of EACH card. The photos go on the back of the cards in the upper left-hand corner on each card. The position is indicated on the back of the member pass. It goes in the same position on the membership card and overlays the Code of Conduct.

Several reminders:

OBSL teams are limited to two (2) players who also play for NCSL or WAGS teams.
The NCSL or WAGS team should be the primary team and the player is multiple rostered on the OBSL team.

The team manager must have the proof of age document before a player signs the back of the two cards and must instruct the player how to sign his/her name. If you don’t follow this simple procedure Paul Breitner is going to sign his name as Joey Breitner and his signature will have to be corrected.

All teams are using the new USYS player passes. The second field on the passes, "Member", will be blank. Adult cards will only print on the blue membership card. There is only one card per adult vs. two for each player.

For new teams, the general procedure is:

  • Get registration forms completed for each player and each adult official and obtain the birth certificates and two photos for each player and one phto for each official.
  • Download the registration database program and corresponding instructions.
  • Check the birth certificates for the child’s legal name and date of birth. Do not confuse the "Date Issued" with the "Date of Birth".
  • On the player registration form for Paul Breitner, cross out "Joey" which mom or dad listed and insert Paul. You now have accurate data for each player.

Since uniform number is not listed on the registration form, the person entering the data will need a list of the jersey numbers. Also, if you have Multiple Roster players, you need both the social security number and the M/R number.

  • Follow the current database instructions. Input the data for each player and each adult official.
  • Print the roster per the current instructions.
  • Print the player passes per the current instructions
  • Tear off the bottom and top margins of the player passes. Glue the photos for each child onto the back of the child’s passes. Have the child sign his legal name (as it appears on the front of the passes) on EACH pass. Do not separate the two passes. Your registrar will do that.

Rostering Process

Your team must be rostered to play in league games and tournaments. That means getting your forms, passes and roster validated by your designated registrar.

There are no separate fees for registration. Olney NCSL and OBSL teams have paid the MSYSA registration fee as part of their league fees and the leagues remit the registration fees for each of their league teams to MSYSA.

When you submit your package to your registrar, have the individual paperwork for each player in alphabetical order as they appear on the roster. For each player have the Member Pass/Membership Card, birth certificate and Membership/Player Registration form plus copy paper clipped together. Also paper clip together the adult pass and registration form plus 2 copies for each team official. You will also have the roster and copies and computer disk containing your team’s database file.

Package Submitted to Registrar

Each team must submit the following for each player:

  • Acceptable Original Proof of Age document
  • Member Pass/Membership Card with photo and player signature on back of EACH - do not separate the 2 cards
  • Membership/Player Registration form signed by
    player and parent PLUS 1 copy

Each team must submit the following for each adult official:

  • Adult registration form signed by team official PLUS 2 copies
  • Blue Adult Membership Card with photo and signature on back

Teams must also submit:
OBSL teams must submit their original roster PLUS 4 copies. Olney NCSL teams and Olney Soccer's OBSL teams must submit their original roster PLUS 5 copies.
Computer disk containing team database file.

When the team roster and each of the player and adult official passes is correct, your registrar will validate the roster and passes and notify you they are available .

Procedures for Roster Changes

Most teams will have roster changes involving adding new players or releasing players who have elected to play another sport or have decided to play in a house program. All teams must maintain a current roster.

Adding a Player

When adding players the process for each new player is the same as when you originally rostered your team except all adds or transfers are printed on your original roster in the next available row (and you can print the player data on the front of the player pass/membership card). Remember there is a slightly different procedure for MSYSA-rostered teams if the player lives in Virginia or the District. Also, if the player is currently rostered to another travel team, you need to obtain a Multiple Roster Player Pass and player number from your registrar.

Submit to your registrar the following for each player being added:

  • Acceptable Original Proof of Age document
  • Member Pass/Membership Card with photo and player signature on back of EACH - do not separate the 2 cards
  • Membership/Player Registration form signed by player and parent PLUS 1 copy
  • Original roster with new player printed in next available row PLUS copies detailed above
  • Any applicable fees if over cumulative roster limit

Player Transferring To Your Team

If you are adding a new player who has been rostered to a travel team during the current seasonal year, his/her former team must release the player before he/she can be transferred onto your roster. The player is entered on your roster by printing the player’s data at the bottom in the Previously Rostered section.

You must bring either:

( two copies of the Player/Team Status form, signed a and stamped by the registrar of the former team in ) the release section to show the player has been released; an original Player/Team Status Form and three copies with the top two lines and Transfer Request section completed and signed by a team official who appears on the first page of your roster; the Player Pass; original updated roster and sufficient copies, and any applicable fees,

or:

( four copies of the Player/Team Status form which b has been signed and stamped by the registrar of ) the former team in the release section to show the player has been released, and with the Transfer Request section completed and signed by a team official who appears on the first page of your roster; the Player Pass; original updated roster and sufficient copies, and any applicable fees.

Criteria For Adding Players

New players may be added during the season provided the transaction complies with league rules. For both NCSL and OBSL teams, a player who is rostered to one league team at the start of the season is ineligible to play for another league team during that season. Both NCSL and OBSL define the start of the season as the date of the first scheduled league game.

MSYSA Fees

The maximum number of players on your roster is detailed in the OBSL Coaches Manual and on the NCSL website. You cannot exceed the maximum roster. There is a $10.50 per player fee for each cumulative player above your maximum roster. If your maximum roster is 18 and you initially rostered 18 players and subsequently have 3 releases and 2 adds, your current roster has 17 players and the cumulative number is 20 and you must pay a $21.00 fee.

There is also a $5.50 fee for each previously rostered or transferred player added to your roster. All payments must be by check payable to MSYSA.

Releasing Player From Your Team

For releasing players, remember the Players Rights. The initiative for releasing players should come from the family as a result of the child not playing on your team. To release a player you need a Player/Team Status Form that is available on the MSYSA website.

This form is given to the family and a parent (or team manager) fills in the top two lines that contain basic player data. The player and parent sign in the boxed area and a parent or the player states the reason for leaving the team. The reason is usually very simple:" Playing lacrosse " / "Not playing soccer " / "Playing in a recreational program ".

A team official listed on the first page of your roster checks the Release Request box, fills in the team data and signs and dates. Make three copies of the status form and submit to your registrar with your original roster and sufficient copies, Player Pass and a stamped envelope addressed to the player’s parent/guardian. Do not mark the line containing the player on your roster. The registrar will annotate your roster to indicate the player has been released. The registrar will keep the player pass and the family may obtain the pass if the player decides to join another travel team.
